Using Forms on ShareSuccess

  • All employee forms are housed in ShareSuccess, in an easy to use and searchable database. To access the forms, sign into Office365 with your District email address and Active Directory Password.
  • Parent/Student/Public forms are housed on the MSBSD website. Links to these forms are available on ShareSuccess in the forms depository.


  1. Navigate to ShareSuccess. If prompted, sign in using your District email address and your Active Directory password. The ShareSuccess landing page will load. Click “Forms” in the top navigation bar.

  2. The Forms library will load:

  3. To locate department-specific forms, hover over “Department.” Click the down-arrow that appears and select the appropriate department(s). The list will filter to only display forms related to that department.

  4. Locate a form using a keyword. Type the keyword in the search box above the forms list. Type enter on your keyboard or click the magnifying glass. Results load below the search box.

  5. Combine the searching tips in steps 3 and 4 to obtain faster results. Enter a keyword and type enter on your keyboard or click the magnifying glass. Filter by “Department” to narrow down the results.

  6. To download the form for use, select the form so that it is highlighted in blue. Click the “. . .” next to the form title. Click “Download.”

  7. When prompted, click “Open” to view the form.

  8. To view Public Forms (for parents, students, vendors, etc.), click “Public Forms” in the left-hand navigation menu.

  9. To view KnowledgeBase documents, click “KnowledgeBase” in the left-hand navigation menu or in the top menu.
