Appreciation Payment Request

Overview:

This form is used for requesting an appreciation payment. This payment will be processed by the Payroll Department on the earliest scheduled payroll possible. If the Appreciation Payment Certificate is being paid to someone who is not an MSBSD employee, please contact Payroll to have them complete the additional forms.

The minimum amount of an appreciation payment is $100, and the maximum amount is $500. Appreciation payments may be submitted for any amount between $100 and $500. The full cost of the appreciation payment, including benefits, will be charged to the school or department’s overall discretionary budget. Verify account codes through Account Inquiry, if they are not in Munis, please submit a New Account Code Request.

Request Process:

  • Select Request Service from the menu on the right.
  • Fill out the form to provide Payroll with information regarding your request for an appreciation payment.
