Navigate to the Tree:
Select MATSU
Select Student
Select Injury report
Use the search fields to bring up the injured student.
Click the Add button and fill in the report fields as fully as possible.
Report Date is the date that the report was completed and may differ from the incident date.
Date is the date of the incident or injury.
Time is the approximate time that the incident occurred.
Initial Reporter is the name of the staff member who reported the incident to the nurse.
Occurred describes where the incident took place.
The Other field is used if Other is selected in the Occurred field.
Click Save - The date and occurred fields are required to save an incident.
Click the Show Detail button (next to Add) and record other incident details .
If an injury occurs on a playground, include the playground activity and surfacing type.
Drop-down menus are provided for most fields. The Incident Cause, Type of Injury, Part of Body Inured and a description of the incident can be written by the witness(s) of the incident. Include witness names and job titles when possible.
The Student Referred to, Student Transported by, Who was notified, and Notification made by fields are completed by the nurse.
The Nurse’s Actions section is completed by the school nurse (or another authorized individual when no nurse is available).

The report will show basic information about the incidents that have occurred at the school. For more information about an incident, the individual records can be accessed by clicking the Date field of the incident.
This opens a new window with the student record, select Show Detail to view the report.

Report U-HE600
The report can be scheduled as a recurring report by selecting the Tree on the left > MATSU > Student > Reports > Summary > U-HE600 - Student Injury Report.
Navigate to Advanced Settings and select how often you’d like to receive the report (monthly recommended).
Open the Notification section and enter your Email address to have it send you a notification. Selecting the Check box will attach the report for easy viewing.
Click Print at the top left to generate the report, and begin the schedule.
