Navigate to: Synergy>Matsu > Student >
Crisis Response
Pull up the student to enter the record for
The first tab shows ALL entered records for reference
You can enter (or view existing) data utilizing either appropriate tab for the incident:
Suicide Intervention
Non-Suicide Intervention
TO ADD A RECORD
:
On the appropriate tab/grid, click:
Add
In the new row input the following:
Date of Incident
Incident#: SCHOOL ACRONYM
followed by three digits. Ex:
WHS001
.
The school acronym is important for record keeping purposes.
Select the
type
: Suicide or Non-Suicide
Click:
Save
Expand the line item/show detail in the grid you just added:
Input all necessary information
STUDENT SAFETY PLAN
Utilize the
Student Safety Plan
tab to input the team members
UPLOAD/ATTACH COPY OF STUDENT SAFETY PLAN

Click on the
Documents
tab
Click:
Add
Upload your document with a category of: Additional Crisis Response Docs
For security reasons, please verify the correct category
WHEN FOLLOW-UP IS NEEDED
:
Use the
Reminder
Feature at the top of the screen to set a reminder:
Click on the Reminder Icon and click:
Add Reminder
Set the Description, Date and Time and click:
Create
Handle with Care Icon
: Automatically Appears within 24hrs
