Adding a Health Condition

Open the Health screen in Synergy.
Find the student you need to add information on.
Select the Health Condition tab
Click Add on the Conditions section to a add a new row.
Enter the appropriate information for the condition:
- Order By - Order in which the student's conditions display
- Condition code - Select from the list of conditions
- Critical/Chronic check box
- Date Entered - Date the condition is entered into the system
- Start Date - Date condition began
- End Date - Date the condtion ended if needed
- Comment - Any comments related to the condition

Adding a Medication

Health Screen in Synergy
Find the student you need to add information on.
Select the Medications tab.
Click Add in the Curent Medication section to add a new line.
Enter the appropriate information for the medication.
- Medication - Name of medication
- Remaining Units - Remaining units for a medication
- Start Date - Date you begin administering the medication.
- End Date - Date you finish administering the medication, if needed
- Dosage Times - Times during the day you administer the medication, the number of doese, and the prescribed amount of medication. NOTE: You can record separate dosage amount for each scheduled time frame for the same medication in Dosage 1, Dosage 2, and Dosage 3 columns.
- Admin Days - Days you administer the medication
- Days Between - Number of days between doses
- IEP - Indicates the student has an Indivudual Educational Plan (IEP) on record
- Route of Admin - How the medication is administered(for e.g, orally, topically)
- Medicaid - Indicates if the medication is billable to Medicaid
- PRN - Indicates if this ia a "taken as needed" medication

Adding Medication Details

Select the medication you want to add details for in the Current Medications section.
Click Show Detail- The student mediciation section displays information from the Current Medications section.
Add the appropriate information for the medication.
- Medication Type - Indicates if the type of medication (for e.g., liquid, capsule, topical ointment, etc.)
- School Provided - Indicates if the school providied the medication (for e.g., ibuprofen, Neosporin, etc.)
- Prescribing Physician - Pysician who prescribed the medication
- Note - Notes about the medication
- Side Effects - Any possible or noticed side effects of the medication
- Administered In - Where the medication was administered (for e.g. health office, classroom, field, gym, etc.)
- Student Health Conditions Related to this Medication - Select the known medical condition related to this medication
View the Current Mediation history for the student in the History section.

Adding a Procedure

Open the Health screen in Synergy.
Find the student you need to add information on.
Select the Medications tab
Click Add on the Curent Procedures section to a add a new line.
Enter the appropriate information for the procedure:
- Procedure - Name of the procedure
- Start Date - Date you began performing the procedure
- End Date - Date you finish performing the procedure,if needed
- Procedure Times - Times during the day you perform the procedure
- Procedure Days- Days you preform the procedure
- Days Between Procedures- Number of days between procedures
- IEP - Indicates the student has an Indivudual Educational Plan (IEP) on record
- Medicaid - Indicates if the procedure is billable to Medicaid
- PRN - Indicates if this ia a "performed as needed" medication

Entering Student Immunizations

Open Health screen
Find the student to update
Select Immunizations tab
Enter the date the student received the vaccination in the Dosage Data section
Save

Entering Immunizations Exemptions

Find the student on Health screen, go to Immunizations tab
Select the record and Show Detail or clcik the arrow to open the details
Enter the date, reason, and expiration date as needed
Save

Religious Exemption

Find the student on Health screen, go to Immunizations tab
Set Exemption and Compliance section
Exempt dropdown, choose Religious Exeption
Check the confirm set and compliance checkboxes.
This will mark all as compliant for the year.
These will be cleared out annually at the end of June in preparation for the new registration process.

Setting as Compliant

Find the student on Health screen, go to Immunizations tab
Select the appropriate exemption value from the Exemption Granted field in the Immunization Record Data section
Save

Recording record data

Find the student on Health screen, go to Immunizations tab
Expand the Immunization Record Data section
Enter the name of the staff member who received or entered the records in Staff Signature
Enter date received or entered
Enter Type of Record
Indicate if there is a parent signature on file
Enter Parent signature date
Select Status of Requirements of Exemption Granted
Complete any needed fields
Save

Health Office VisitsSee More

Entering Basic Health Office Visit information, record any health incident that requires a visit to the nurse's office. You can add a short entry and then add details later as needed, or use the ADD Wizard to add all the details at once.

Health screen, find student record
Entries section, Click Add (for short entry) or Add Wizard (to fill in all details)
When using short entry Add
- Time In - Enter the time the student arrived
- Time Out - Enter the time the student left
- Referred By - Staff member who sent the student to health office
- Health Code - Reason for visit
- Staff Name - Health staff member who treated the student
- Save
When using Add Wizard
- Disposition - Student's disposition at the end of the visit
- Referred By -Staff member who sent the student to health office
- Follow Up - Date to follow up with the student
- Parent Contact attempted, or Parent Contact made
- Visit Comment 1- Comments about the visit
- Visit Comment 2 Additional comments about the visit
- Vital signs, Clinical Code and Health assessment can be added if needed
- Save
There is an Accident details tab in this mode where you can input details if the reason for the visit pertains to an accident. There is a print report button here that you can print the report once you have entered the needed details.
Private Comments and Health Documents, use the Private tab on the Health screen.
- Comments section has Add and Add Wizard options
- Document section when you click Add will open your computer File Explorer window, find the document you need to add.
- Select the Doc Category
- Enter a description in the Doc Comment field, the system defaults to the file name
- Save

To enter Health screening information

Navigate to the "Health Screen" screen
Select the screening tab you need to enter information on
Use Add or Add Wizard button where indicated
Complete the needed fields
Save

To enter Health screening information by Grade

Navigate to the "Health Screen By Grade" screen
Select the appropriate grade,click Find
Student list will populate
Select the screening tab you need to enter information for
You can use the filter section to sort by date range, gender, or sort by option, then click apply filer
Highlight the student, show details and complete the needed fields
Save

To enter Health screening information by Section

Navigate to the "Health Screen By Section" screen
Select the appropriate Section, click Find
Student list will populate
Select the screening tab you need to enter information for
You can use the filter section to sort by date range,and you can check the box to hide students with a screen date within the date range, then click apply filer
Highlight the student, show details and complete the needed fields
Save

Recording Medication or Procedure Administered to a Student

Medication/Procedure icon on the Tasks list or navigate to Synergy SIS>Health>Medication and Service Monitor
Select column options in the Show/Hide Columns section you want to display and deselect the options you do not want to display.
Select Show all prior unprocessed tasks to show the past records per the Number of days to retain unprocessed health medical tasks value.
Enter the appropriate information in the columns
Date and Time - Synergy auto populates the field with today's date and the scheduled time. This field is editable
Units - Synergy auto populates with the medication units defined for this medication on the Health Screen.
Administered - Select Administered if the medication/procedure was administered as scheduled. If not, select the appropriate reason, such as Absent.
Total Admin Time - Amount of time it took to administer the medication/procedure.
Save
Show Detail to add any Administration Notes.
Save

Mass Updating Medication or Procedure Administered to a Student

Medication/Procedure icon on the Tasks list or navigate to Synergy SIS>Health>Medication and Service Monitor
Select the appropriate option in the Administered field.
- Absent
- Administered
- Dosage Adjustment
- No Show
Select Staff member who administered the medication/procedure in the Administered By field.
Click Mass Update Students

Viewing Previously Administered Medications and Procedures

Medication/Procedure icon on the Tasks list or navigate to Synergy SIS>Health>Medication and Service Monitor
Select the History tab. This will display the last 30 days.
click the Student's name to display the student's Medications tab on the Health screen.

Creating a Health Plan
Printing the Individual Healthcare Plan

Creating a Health Plan

Students with long term or chronic medical issues must have a healthcare plan on record. Healthcare plans help assure consistent, safe healthcare for the student and provides documentation regarding the services provided by the school.

Synergy SIS> Health>Individual Healthcare Plan
Find the appropriate student record
click Add in the Healthcare Plan section. Ths Healthcare Plan Add screen will open.
Enter the needed information.
- Plan Name - Name of the healtchcare plan
- Plan Start Date - Date the plan goes into effect
- Plan End Date - Date the plan ends
- Written by - Health staff memeber who created the plan
- Medical Diagnosis - Medical condition being addressed
- Nursing Diagnosis - Current health status of the student and defines what the nurse plan to contribute
Click Add on the Intervention section to add a new line.
Enter the Intervention Code and Intervention Description. An intervention is any treatment performed to reach a goal or expected outcome.
Click Add in the details section to add a new line.
Add the needed information
- Nursing Assessment - Background information including health history, current health status, self-care skills and needs, psychosocial status, and health issues related to learning
- Nursing Plan - Action plan to treat or manage the student's illness or condition
- Medications/Side Effects - Any medications and possible side effects that may need to be managed
- Expected Student Outcomes - How the student is expected to participate and what they are expected to do
Click Save

Printing the Individual Healthcare Plan

Parents and guardians must review and approve the healthcare plan before putting the plan inot action. Synergy SIS enables you to print a boilerplate cover letter and a form detailin ght healthcare plan to send to the parents. You can also translate the letter for the parents into th eappropriate language.

Synergy SIS>Health>Individual Healthcare Plan.
Locate the appropriate student record.
Click Show Detail link
Select the Merge Document to print the cover letter if needed.
Select the Merge Lnaguage to set the language of the letter if needed.
Print Report

Synergy prints the Healthcare Detail Plan report and Cover Letter if you selected a merge document.

Viewing Student's Medical History

Viewing Student's Medical History

The Health History tab displays the student's health history at school. It displays all records of health incidents, conditions, medications, and procedures the student has had while attending this school.

Synergy SIS>Health>Health
Locate the appropriate student health record.
Select the Health History tab.
Click the Show Details to see the details of any historical incident, medication, or procedure.

ReportsSee More

This section will cover:

Reports

Report types

Individual - 200 level reports are individual reports. These print information for a single student per page and can be printed for multiple students at once.
List - 400 level reports are List reports. These show details for multiple values, such as students or teacher, on one page.
Summary - 600 level reports are Summary reports. These provide numerical totals for each category specified.
Extracts - 800 level reports ar eExtract reports. These export information from Synergy SIS into a text file that can be used to import data into another program.

Each report has a Report Interface screen where you can select specific report options. The Report Interface has these five tabs

You can also run reports from any Synergy screen that focuses to a student or from Find results at Synergy SIS>Student>Student using the Actions icon. Synergy Actions includes most reports that have a student section in the Sort/Output tab.
